Your Role on the Hospice Team
As the Referring or Attending Physician, you will be an integral part of your patient’s care team and of Homestead Hospice's interdisciplinary team. You will have the most significant role in the determination and delivery of the patient’s medical hospice care.
The Referring/Attending Physician
Research has shown that hospice can be a way of offering more support and improved care to patients during the terminal phase of an illness. When the Physician refers a terminally ill patient to Homestead Hospice, they can continue to retain control of their plan of care if desired. The Physician will be contracted to bill for the services provided related to the patient’s terminal diagnosis.

Once a referral is received by Homestead, a hospice Nurse will meet the patient within 24-hours for an assessment to determine hospice eligibility. Upon hospice admission, the patient is provided a multitude of resources and support, with immediate communication for care planning and scheduling. The Attending Physician can choose to remain as the patient’s primary physician and can work with the Homestead team to review care plans and discuss conditions and prognosis.
As the Referring or Attending Physician, you are in a unique position to provide end-of-life care, which includes recognizing the need for and recommending hospice care when appropriate.
