COMMUNICATION

 

  • Every hospice team member will communicate with designated staff and log communication in the Homestead Hospice Communication Binder.
  • A Communication Binder will be placed at every nurse's station for easy access to your facility's team.
  • Our Community Relations Managers meet with your team and attending physicians. They review all patients to ensure we are meeting their needs, and communicate that to their families.
  • Our Customer Service Nurse is available daily to answer your questions and concerns.
  • Our nurses communicate with patients' families after every visit.
  • Our Community Relations Manager meets with your facility's billing department monthly to review hospice billing.
